The Deliverable

What your client receives.

Every referred client receives the in-home Home Safety Assessment and the written Home Safety Report that follows it. The report documents each area of the home, explains why each risk matters, and lays out a recommended scope of work in plain language. The pages below are drawn from a sample report. Names and identifying details have been changed.

Bathing area page from a sample Home Safety Report with observations, reasoning, and recommended actions
Each flagged area receives a full page of observations, reasoning, and recommended actions.
Recommended scope of work page from a sample Home Safety Report, sequenced by priority
The recommended scope of work is sequenced so the most protective items come first.

These four pages are an excerpt. Ask for the complete sample when you reach out, and we will send it to you.

Clinical Scope

What we handle in the home.

Every intervention is sized and positioned to the specific client, not placed to a generic diagram.

Bathroom

Fall Risk Mitigation

  • We install structural grab bars anchored into studs, blocking, or load-rated hardware, positioned to the client's grip and reach.
  • We convert tubs to walk-in showers, including zero-threshold roll-in configurations.
  • We raise toilets to comfort height and add transfer support and slip-resistant surfaces.
Mobility

Access and Transitions

  • We install straight, curved, and outdoor stairlifts.
  • We build wheelchair ramps and install vertical platform lifts.
  • We widen doorways and modify thresholds.
Post-Discharge

Home Readiness on Your Timeline

  • We schedule assessments within one to five business days, and discharge timelines are prioritized.
  • Installation can be phased, with the priority safety items completed first and the larger scope to follow.
  • We coordinate directly with family when the client is not yet home.
Daily Living

Support for Daily Activities

  • We install accessible cabinetry and pull-down shelving.
  • We replace round knobs with lever hardware and fixtures that ask less of the grip.
  • We add task lighting at the zones the client uses most.
How a Referral Works

The handoff stays clean.

A referral should not add to your workload. Here is how it goes.

1

Connect

Send the form, or reach us by phone, text, or email. We also work from a first-name introduction when you have given the family our number.

2

Response

We respond the same or the next business day and reach out to the client or family directly. We will not follow up with you unless we need something specific.

3

Assessment

A Certified Aging-in-Place Specialist completes the Home Safety Assessment in the home, at no charge for clinician-referred clients, and prepares the written Home Safety Report.

4

Install

Work is completed on a timeline that fits the situation. Documentation is sent to you when it is relevant to ongoing care.

How we work with you

Occupational and Physical Therapists

Specify a grab bar at a particular height, orientation, and length, and that is what gets installed. If something raises a structural, clearance, or code concern, we call you before we deviate.

Discharge Planners

Assessments are scheduled within one to five business days, and discharge timelines come first. Priority safety items are installed before the client comes home, with phased work after.

Home Health, Home Care, and Care Managers

We show up on time, document the scope, and follow up after the install. We aim to be the kind of partner you can refer to more than once.

Refer a Client

Two ways to connect us.

Choose whichever fits your workflow. Both receive a same or next business day response.

Option 1 · Direct Contact

Call, text, or email.

Mention that you are a clinician when you reach out, because we route those calls differently. If the client is on a discharge timeline, say so up front and we will adjust.

Phone · Ventura County (805) 500-0801
Phone · Los Angeles Area (213) 799-5399
Email contact@aceaccesshomes.com
Option 2 · Referral Form

Send the details in writing.

If a form fits your workflow better than a phone call, use the referral form at the top of this page. Include only the client information you are authorized to share. We can also work from a first-name introduction, with the client or family contacting us directly.

Questions Clinicians Ask

Frequently asked questions.

If your question is not here, call or text us at (805) 500-0801 and we will answer it directly.

Is the Home Safety Assessment really provided at no charge?

Yes. The Home Safety Assessment is normally $329. For clients referred by a clinician or care professional, we provide it at no charge, and it carries no obligation. The family keeps the written Home Safety Report whether or not any work moves forward.

What does the Home Safety Assessment include?

A Certified Aging-in-Place Specialist walks every room of the home, observes how the client moves through the space, takes measurements, and documents risk area by area. The visit produces the written Home Safety Report, which ranks each area of the home by severity and priority and lays out a recommended scope of work in plain language.

How quickly can you see a client on a discharge timeline?

We respond the same or the next business day, and assessments are scheduled within one to five business days. Discharge timelines are prioritized, and priority safety items can be installed before the client comes home. If a discharge date is close, call us and tell us up front.

Will you follow the specifications I set for my client?

Yes. If you specify a grab bar at a particular height, orientation, and length, that is what we install. If a structural, clearance, or code issue makes the specification unworkable, we call you before we deviate from it.

What client information do I need to share?

A first name, a phone number, and a city or town are enough for us to make contact. Share only the information you are authorized to share. We never share clinician or client contact information.

Is Ace Access Homes licensed and insured?

Yes. Ace Access Homes is a licensed California contractor, CSLB #1087045, and the company is bonded and insured. Assessments are performed by a Certified Aging-in-Place Specialist, a credential issued by the National Association of Home Builders.

Which areas do you serve?

We serve families across Ventura County, the Conejo Valley, and the greater Los Angeles area, including Ventura, Camarillo, Oxnard, Moorpark, Simi Valley, Thousand Oaks, Westlake Village, Newbury Park, Pasadena, Burbank, Glendale, Los Angeles, Malibu, and Santa Barbara. If your client lives near but outside these areas, call us at (805) 500-0801 and we will tell you honestly whether we can help.

Still Evaluating?

Come walk a job with us.

If you are deciding whether to refer clients to us, the fastest way to know is to see the work. Paul will meet you at a recent install site, or sit down over coffee to talk through how we operate. There is no pitch and no pressure.
